we want you to have the best possible experience with the least amount of
hassle. Follow the tips below to schedule your first appointment.
- Obtain a referral/prescription from your doctor
Sometimes it is possible to just call your doctor to receive a referral without making an appointment.
*This makes it possible for us to bill your insurance for the treatment. You can see us without a referral, but the chances that your insurance will pay is unlikely. *Some insurances require a pre-authorization, so if you have any questions please call

- Call and make an appointment
1st Appointment
Once you've received a referral for physical therapy, please contact our office to schedule an appointment. Your initial appointment will take 60-90 minutes.
- Arrive 15-20 minutes early to fill out paperwork. Or arrive on time with paperwork already filled out. Look below for printable paperwork.
- Have insurance cards/co-pay
- Wear comfortable clothing
